NMC guidelines for MBBS abroad 2026 for Indian students

Every year 25,000+ Indian studentsgo abroad for MBBS. But after NMC's new rules introduced in 2021, more than 50% of students are at risk of their medical degree being INVALID in India. At Medupreneurs, with 20 years of experience in global medical education, we have seen hundreds of students in distress after returning because unscrupulous agents sent them to non-compliant institutions.

This guide will save your medical career and family investment. Read it carefully before you pay any university fees.

NMC Circular Advisory Warning

“Students graduating from non-compliant foreign medical institutions will be held ineligible for licensing exams (FMGE / NExT). The onus of verifying university compliance lies solely with the student.”

What is the NMC FMGL Regulation?

The National Medical Commission (NMC) officially gazetted the “Foreign Medical Graduate Licentiate (FMGL) Regulations, 2021” on 18th November 2021. This is now the binding law for any Indian student who takes admission in a foreign medical university on or after 18 November 2021.

If your university fails to satisfy even one clause of this regulation, you CANNOT appear for FMGE / NExT, cannot register for CRMI internship in India, and cannot obtain a license to practice medicine in India.

The 7 Non-Negotiable NMC Rules for MBBS Abroad in 2026

This is your master checklist. If any single rule fails, immediately reject that university:

1. NEET-UG is Mandatory — Without Any Exception

You MUST qualify NEET-UG before departing India. Qualified NEET scores for students going abroad are valid for 3 years from the declaration date. Even if a foreign university offers you admission without NEET qualification, NMC will summarily reject your degree. No agent or university authority can bypass this statutory requirement.

Eligibility Criteria: Minimum 50% in PCB for General category (40% for SC/ST/OBC) + NEET Qualified in the current or preceding 2 years + Minimum 17 years of age by 31st December of the admission year.

2. The 54 + 12 Rule — The Fundamental Duration Benchmark

Your foreign medical course must consist of a minimum of 54 months (4.5 years) of theoretical and clinical academic study + 12 months of compulsory rotatory internship = Total 66 months (5.5 years) in ONE single institution.

  • 5-Year MBBS without internship? INVALID in India.
  • 4-Year MD program (Philippines BS+MD style)? INVALID in India.
  • Any program total under 5.5 years? INVALID in India.

3. Internship MUST Be in The Same Country & Same University

You must complete your full 12-month clinical internship in the SAME foreign medical university and affiliated hospital where you pursued your academic theory. You cannot do your primary internship in India or in another third-party nation.

Note: This rule has made many split-island Caribbean models and offshore Philippines programs non-compliant for Indian licensure.

4. No Split-Campus or Two-Country Programs

NMC has strictly prohibited dual-campus or split models where you study initial pre-med or basic sciences in one country and clinical years in another country. For example, packages offering “2 years in Philippines + 2 years in USA clinicals” without satisfying full in-country graduation are NOT valid for practicing in India.

Your entire medical education + clinical internship must be completed in ONE country, ONE institution.

5. 100% English Medium Throughout — Not Bilingual

The medium of instruction must be entirely English for all years of study, including lectures, laboratory practicals, bedside clinics, and examinations. Certain universities in Russia or China operate bilingual courses (teaching the initial 2-3 years in English and subsequent clinical years in Russian/Mandarin) — this is NOT acceptable to NMC.

Always obtain written and apostilled university confirmation stating: “The medium of instruction is 100% English for all years including clinical clerkship and internship.”

6. Mandatory Subjects Must Be Covered

Under Schedule I of the FMGL Regulations, your curriculum must encompass: General Medicine, General Surgery, Psychiatry, Paediatrics, Obstetrics & Gynaecology, Anaesthesiology, Ophthalmology, Orthopaedics, ENT, Dermatology, Emergency Medicine, and Community Medicine.

If the foreign medical curriculum omits any of these mandatory clinical subjects, you will be deemed ineligible for Indian licensing.

7. University Must Be Legally Recognized + WDOMS Listed

Crucial Fact: NMC does notpublish an “approved list” of foreign medical colleges. What NMC mandates is:

  1. The university must be actively listed in the World Directory of Medical Schools (WDOMS) at wdoms.org.
  2. The institution must be legally authorized by the national accreditation authority of that country to grant degrees.
  3. The degree must grant you full eligibility to practice as a licensed medical practitioner in that awarding country on par with domestic citizens.
  4. No online degrees. All training must be conducted physically and offline.

What Happens If You Join a Non-Compliant University?

The consequences are devastating for students and their families:

❌Degree Unrecognized

Your MBBS/MD cannot be registered with any State Medical Council in India.

🚫Barred From NExT / FMGE

You will be permanently disqualified from appearing for the Indian licensing exam.

💸Loss of 6 Years & ₹35+ Lakhs

Years of hard work, family savings, and career momentum lost irreversibly.
